Recruiters and hiring managers come only with general requirements. They cannot describe what will the  candidate do together and therefore they cannot choose someone who will be able to do it.

Managers do not know how to conduct interviews

They ask very general questions and cannot articulate what they want the candidates to do. In combination with the candidates who are confused as well, this cannot bring any good.

Managers hate recruitment

You want a new employee as soon as possible but managers, on the other hand, do not take the recruiting process as a priority. It is rather a necessary evil for them and they are trying to delay it as much as possible.

You have incompetent managers and recruiters

Bad managers and recruiters tend to chose bad employees. Competent people in the roles of managers and recruiters are essential for successful recruitment.
